**For Participants of a programme - As above, plus GP information, Physical Activity Readiness Questionnaire responses and Participant Questionnaire responses.

Purpose of collecting data

To enable communication between parties; provision of business to business/organisation services; facilitating the safe discharge of services including fufilling statutory responsibilities and obligations.

Any health data collected is used to ensure participants are suitable for the programme and, where necessary, to make adjustments to the programme for indviduals.  In exceptional circumstances participants may be deemed physically unsuitable or may be required to make contact with their GP before joining a programme.  

Questionnaire responses are collected and collated to demonstrate effectiveness of a programme and measure changes over the course of a programme.

All responses will be treated in the strictest of confidence.

At One With Nature Ltd is not a medical organisation and any information shared will not be used to diagnose any conditions.  In very exceptional circumstances and where there is a significant and justifiable cause for concern about the immediate welfare of an individual then information will be shared with your GP.
